MTM is a service provided by your pharmacist for individuals with multiple chronic conditions like asthma, diabetes, hypertension, and congestive heart failure. MTM is designed to optimize your medication therapy and improve your health and wellness outcomes.
When MTM is appropriate, a Starwood Pharmacist will plan for a phone or in-person consultation and identify medication-related challenges to determine the best plan for improvement and help you set goals. Starwood Pharmacists will also communicate with your healthcare providers to improve your health.
